NOTIFICATION OF CHANGE OF A NAME


A) By Marriage:
a. The Registered Owner shall present the completed Form No.4,
b. A Notification of Change form
c. A Computerised Certificate of Marriage and a copy.

b) By Divorce:
a. The applicant shall present the completed Form No.4,
b. A Notification of Change form
c. A decree absolute certificate.
 
c) By Affidavit
a. The applicant shall present the completed Form No. 4
b. A Notification of Change Form
c. The original and copies of his/her Computerised Birth Certificate and Affidavit
 
d) By Deed Poll:

a. The applicant shall present the completed Form No. 4
b. Notification of Change form
c. The original and a copy of the Deed Poll A fee of $35.00 is applicable for the Notification of a Change of Name

CHANGE OF ADDRESS

Applicant must be supported by documents as proof of address such as a recent Utility Bill or in the case of a rental, a copy of the Lease/Rental Agreement for the rental premises identified and bearing the name of the Applicant. If the utility bill or Lease/Rental identification (ID) MUST be submitted.
