APPLICATION FOR ENDORSEMENT FOR CLASS 4 (HEAVY) AND CLASS 5 (EXTRA HEAVY GOODS VEHICLE)

The applicant shall be at least twenty-one (21) years of age and shall be the holder of a driver’s permit of the previous lower class of motor vehicle (i.e. for Class 4, the applicant must be a holder of Class 3, and for Class 5, the applicant must be a holder of Class 4). The applicant should be the holder of the specified Class of Driver’s Permit for not less than one (1) year prior to the application and must produce the following:  

1. A completed application  Form No. 4 

2. A current Driver’s Permit 

3. A valid proof of address – (utility bill or a copy of the Lease/Rental Unit Agreement) See full listing of Proof of Address Requirements 

The accepted forms demonstrating proof of address include a cable bill, home internet bill, electricity bill (T&TEC), OR water bill (WASA). If a customer does not have a utility bill in their name, he or she is required to produce a letter of authorization from the owner and a copy of their national identification
 

4. A completed Form No. 12 “Medical Test for Driver’s Permit” 

5. Four (4) passport-sized photographs, with a full-frontal view of the face (green background) 

 
When approval is obtained, a Provisional Permit is issued at which time, the following fees are required to be paid:
